Generally a nurse or a phlebotomy technician performs venesection, also known as phlebotomy. A vein in upper forearm (antecubetal fossa) is used for blood collection mostly. Blood gas measurements use arterial blood. Capillary bed is used for smaller quantity of blood and a capillary puncture or skin puncture is the method used. Point of care tests use the minimum quantity of blood and are performed by capillary puncture.

Venous blood can be collected in many ways. Venipuncture methods can vary depending on the test requirements, age, and physical condition of the patients. The sites to use are those in forearm, wrist, or ankle but the common is forearm veins. This is because forearm veins are generally more fuller and larger than other veins. Three main veins in the forearm are cephalic, median cubital, and median basilic.

Blood may be obtained directly from a vein using an evacuated tube and needle collection system or by using a sterile syringe and needle. Vacuum tubes were first marketed by BD (Becton, Dickinson and company, BD Vacutainer system). An infusion or butterfly set can be used in combination with the evacuated tube method.

A double ended hypodermic needle is used to puncture the vein in the evacuated tube method, where the shorter end of needle is covered by the holder for phlebotomist safety. One end of the needle is inserted into the vein and the other end is partially attached to a rubber stopper of a special vacuum tube. The rubber cap of the holder is pierced by the second needle when the vacutainer test tube is pushed down into the holder, and this creates a pressure difference in the blood volume and the blood is forced through the needle into the tube by the vacuum present in the tube. Next sample is collected in the same way after removing the tube and inserting another tube. The presence of suction in the tube makes it important to remove the tube before withdrawing the needle from the vein. Evacuated tubes are used for blood specimen collection as per standards set by The Clinical and Laboratory Standards Institute (formerly NCCLS).

In patients with difficult veins the use of a syringe and needle system is made for collection of blood. Vacuum of BD vacutainer tubes may cause collapsing of smaller veins in some patients, and in this case use of syringe and needle is preferred. In case of collection for babies, and small children with small veins, for obese patients with hard to find veins, for patients receiving intravenous therapy with scarred veins, and for veins other than those in antecubital fossa (wrist, back of hand, ankle, foot, or scalp), an infusion set called butterfly is often used.

Phlebotomist can perform multiple duties as per the requirement of the work setting. Phlebotomist is primarily concerned with assisting doctors. Coordinating with nurses may be one of the other duties. Blood sample collection or tissue samples from patients is also the duty of a phlebotomist as a hospital technician.

The phlebotomist ensures that a patient is appropriately identified before samples are taken. Part of the hospital technician’s duties as a phlebotomist is to correctly puncture or incise a patient’s skin to take specimens. The phlebotomist is responsible for the amount of blood or tissue taken should be enough for the specified test. Another aspect of the hospital technician’s duties as a phlebotomist is that samples are properly labeled. The technician also ensures that properly labeled samples are delivered to the laboratory on time.

To achieve these competencies, training from a certified program, or on job training, or cross training from other healthcare profession can be completed by the phlebotomists. For patients with difficult veins venipuncture using syringe and needle is used. Vacuum tube preparation procedures which are common for both methods are followed for this method. Foaming or rupture of the cells is avoided by gentle expelling of blood into a collection tube after withdrawal.

Venipuncture using the infusion set
For veins other than those in the antecubital fossa, for coagulation studies, for babies and small children with small veins, obese patients with hard to find veins, and for collection from scarred veins, a syringe and needle or an infusion set (Butterfly) is used. For a patient with small, fragile, rolling veins or in wrist, back of the hand, ankle, foot, or scalp, the infusion set is used. Winged infusion needles are preferable to standard venipuncture needles when drawing from tiny veins because they are commercially available in small gauges (25 gauge). Pediatric patient collections make use of infusion set. Attachment of the infusion set is done to an evacuated tube holder or it is attached to sterile syringe.

Venipuncture from existing intravascular devices or indwelling lines
Devices like indwelling lines or intravascular devices are used for vascular access for extended period of time for administration of therapeutic blood products, or for infusion of fluids, medications, or parenteral nutrition solutions. It is possible to collect venous or arterial blood samples from these devices, but this is only done by well trained experienced phlebotomists. Infection and septicemia are serious consequences in an immunosuppressed patient; strict asepsis control protocol is required for collection from intravascular devices. Each healthcare facility has its own policy and procedure for collection from indwelling lines or intravascular devices.

A special training program for phlebotomists is necessary for drawing blood from intravascular devices. An intravascular device consists of a silicon catheter, and a self healing silicon septum encased in a metal/plastic ports. It is surgically implanted in the patient using local or general anesthesia, the catheter is tunneled through subcutaneous tissue to a major blood vessel and the portal is secured to the fascia under the skin. The device is accessed by needle puncture through the skin into a port or through a diaphragm. For venous port catheters are placed in a vein. The decision to use an intravascular device for obtaining a blood specimen is made by attending physicians.

Venipuncture from Heparin lock
Indwelling ringed butterfly needle makes up a heparin lock system. Administration of medication intravenously or collection of blood samples can be done in heparin lock for 36-48 hours. Multiple punctures can be eliminated, veins of patient can be saved and trauma to the veins can be decreased by using this device. The diluted heparin solution is injected into the tubing and held inside by a plug at the end of the butterfly line.

Strict adherence to infection control procedures like using antibiotic ointment and careful monitoring for inflammation is needed because a needle is a foreign body placed directly into patient’s vein. Before using sample for analysis a waste specimen of 2-3 ml is withdrawn and discarded to remove heparin solution. A phlebotomist must be specially trained and educated to withdraw blood from heparin lock.

Blood collection for culture
The skin is cleaned three times with povidone iodine solution in a concentric outwardly moving circle before collection to get a sample free from contamination. An alcohol pad is used to remove povidone iodine. Phlebotomist uses gloved finger and follows aseptic precautions. Culture bottles are used for collection of blood using evacuated system. Sterilized and dry container tops are used during collection of blood.

Patient and donor (during blood banking) interaction should be cordial for phlebotomists. A good bedside manner is an important trait for a phlebotomist comparable to doctors and nurses. Phlebotomists should not be upset while drawing blood and other fluids from patients. Finger sticks for obtaining minute samples of blood is also a competency for phlebotomists. College Grant Money Is Available From Many Sources

Not many students or their parents can afford to pay the entire cost of college. Costs at a public university now average almost $10,000 per academic year. College grant money provided by federal and state governments, along with many private institutions, can help. And unlike loans, grants represent money that never has to be reimbursed.

The starting point for most federal grants is the Pell Grant. This is based solely on economic need. A student whose family earns no more than $20,000 per year can qualify for a $4,000 annual grant if attending college full time.

There are federal grants available that are based on economic need and merit. The Academic Competitiveness Grant for freshmen and sophomores and the National SMART Grant for juniors and seniors are two such grants. Students have to maintain a minimum GPA of 3.0, and be enrolled in select majors, mainly engineering, science, and mathematics. Certain foreign language courses and liberal arts degrees may be eligible, as well. A third grant, the Federal Supplemental Educational Opportunity Grant, is based completely on financial need. All three grants require that students also receive Pell Grants.

Every state has a variety of college grants available. Some are based on economic need, while others are for students with specific majors, such as nursing or teaching. Still others are based on ethnicity, or commitment to work within the state in a specific area. One example of this is a grant offered for teachers who want to work on state tribal lands. There are also state grants for older students or for displaced workers. State grants are usually reserved for residents of the state, but some are open to anyone attending college within the state. There are even some reserved for out of state students who wish to transfer to a university within the state.

Many states and private organizations have grants available for students pursuing degrees in critical areas. These include, but are not necessarily limited to, technical areas, teaching, nursing, and physical therapy. Women, especially minority women, can also find grants in areas historically dominated by white males. These are usually in the fields of law, journalism, and engineering.

Students who belong to special groups can find that some states have grants specifically for them. Several states have college grants available for those who are exiting the foster care system. Many states also have grants for those who have parents who were law enforcement officers killed or disabled on the job. Ancestry can also be a source of grant money, as in a grant available for women descended from Confederate Army soldiers.

Most grants, other than those offered by private organizations, require the completion of the Free Application for Student Aid, also known as the FAFSA. Application can be made online or at most university financial aid offices. The FAFSA should be completed as early in the year as possible, even though the deadline is June 30. Many state grants have earlier deadlines, while others are limited to funds on hand. Those applying the earliest have the best chance of securing larger grants.

Financial need is an unacceptable reason to deny anyone a college degree. With more areas now requiring a college education, it should no longer be considered a privilege reserved only for the affluent.
